Deadly Lifeboat Drill -Offshore Drilling Platform
What Happened?
The MODU had just completed it's scheduled lifeboat launching drill. When the lifeboat was retrieved, it was being lifted back to its stowed position. As the lifeboat was raised, the winch was stopped by the limit switch 1-2 feet below its final stowed position. At this point the aft hook latch (rear of capsule) failed to open,this caused the lifeboat to pivot through 90 Deg. transferring the entire load to the bow hook (front),this hook failed in turn,resulting in the release of the lifeboat to the sea. The lifeboat fell approximately 60 feet to the water at an inverted angle (upside down) along with its seven passengers. One crewman lost his life, 6 others were injured.
